Items needed for enrollment are

• Proof of residency--real estate contracts, certified rental/lease agreements and current utility bill
• Parents must provide three (3) different emergency telephone numberst at the time of enrollment
• Please bring a photo ID
• Updated shot record
• Birth cerfificate
